What Else You Need to Know
Our regular office hours are Monday – Friday, 8 am – 5 pm.
In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Korunda Medical Institute.
At Korunda Medical Institute, we accept Medicare, workers’ compensation, auto insurance, and most commercial plans. Please contact your insurance company or you may call our office at (239) 431-6464 to find out if we accept your plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, Discover, and American Express.

If one of our doctors suggests a specialist evaluation or diagnostic testing that requires a referral, we will make every effort to provide this for you as soon as possible. Some referrals require insurance preauthorization and cannot be processed immediately. Insurance guidelines prohibit us from backdating referrals. We require 3 business days for the processing of routine referrals.
Physical Therapy: You will need a referral from your physician or medical provider (primary care doctor, nurse practitioner, orthopedic surgeon, podiatrist, chiropractor, etc.) for insurance to pay for your physical therapy treatment. Korunda Medical Institute will accept an initial referral by fax or phone from your provider, and we will need you to obtain the needed signed order prior to your appointment or you may bring in the signed referral form into our office.

- Physicians Regional Healthcare System (Naples, FL)
